Client Heartbeat [Paid Only]
Client Heartbeat is a pricier survey option starting at $25/month. Honestly though, since the basic version only allows for 50 contacts, you’ll probably want the Gold version at $57/month.
That’s a pretty big price tag compared to the other survey creator tools we’ve been looking at. However, Client Heartbeat does offer some much more advanced reporting features.
You’ll get alerts to warn you of unhappy customers, see how your customer satisfaction compares against your industry benchmark, as well as a testimonial widget that can display endorsements on your website.
Survey Gizmo
Survey Gizmo creates some snazzy looking surveys, and they’re a great choice, even with the free version, so long as you don’t plan on getting a ton of respondents.
Survey Gizmo has a limited free version hidden towards the bottom of their pricing page. The free version offers:

This lack of restrictions makes it a perfect option for bigger businesses who are constantly looking to survey large audiences. One limitation of this tool is that it can be difficult to export data unless you upgrade to their Pro version. Be sure to keep that option in mind before launching a massive feedback campaign.
Google Forms (Web)
Google Forms is the fastest, most intuitive way to build a form. Adding and editing form fields in the Google Forms editor is noticeably faster than using any other app, with question types updating as you start typing (for example, Google Forms recognized an open-ended question as we were typing and automatically changed the answer field from multiple choice to a textbox). You can even reuse questions previously added to other forms to speed up the process.
While Google Forms is technically a form builder, you can also use it to create surveys, polls, or even quizzes that get automatically graded. Collect short- and long-form questions, use linear scales to capture sentiment, or upload several images to let people choose from multiple options in a grid.
Google Forms offers 11 different field types—all of the major types you’d expect in a form or survey tool. It also has simple data validation checks for text fields and conditional logic that serves different questions based on previous answers. You can’t accept payments through a Google Form, but you can accept uploaded files and save them right to Google Drive.
If you need to collaborate with teammates on a form, you can share it with your colleagues so they can add and edit form fields too—even in real time.
